The European Society of Cardiology, in collaboration with the European Renal Association, has recently unveiled its inaugural joint guidelines for addressing cardiovascular disease (CVD) and chronic kidney disease (CKD). These comprehensive directives, featured in the European Heart Journal and introduced at the ESC Congress 2026, aim to streamline the management of these interconnected conditions.

Chronic kidney disease, affecting an estimated 100 million individuals across Europe, is characterized by kidney structural or functional abnormalities persisting for at least three months, with significant health implications. This condition substantially increases the risk of various cardiovascular complications, highlighting the critical need for a unified treatment approach.

Enhancing Early Detection and Risk Mitigation

The newly introduced guidelines underscore the importance of early identification and systematic risk evaluation in patients with intertwined cardiovascular and kidney conditions. Associate Professor Kevin Damman from the University Medical Centre Groningen, Netherlands, emphasized the profound impact of both diseases on patient health and lifespan. He noted that CKD can accelerate CVD progression, and vice versa, leading to earlier onset of cardiovascular events and the need for dialysis. However, recent advancements offer effective treatments to substantially reduce these complications.

Professor William Herrington of the University of Oxford, UK, and Task Force Chair, highlighted the guidelines' objective to enhance the utilization of kidney function and urine albumin testing in individuals with CVD. By improving screening protocols, more at-risk patients can be identified, allowing for the tailored application of appropriate treatments for both CKD and CVD. The 'STAMP on CKD' acronym—Screen, Triage, Address CKD Risk, Modify CVD management, and Plan health services—serves as a framework for these efforts. This framework initiates with comprehensive screening of all CVD patients for CKD using blood and urine tests, followed by accurate assessment of kidney failure and CVD risk through validated scoring systems.

Integrated Treatment Strategies and Patient-Centered Care

The guidelines advocate for the early adoption of proven, cost-effective therapies to slow CKD progression and reduce cardiovascular events. Associate Professor Damman specifically mentioned the crucial role of RAS inhibitors and SGLT2 inhibitors, alongside statin-based treatments, in achieving these benefits. The recommendations also detail modifications for CVD management in CKD patients, including guidance on medications for those with reduced kidney function who may struggle to process standard treatments.

Effective planning of health services is crucial to ensure that high-risk patients receive timely access to specialized care from cardiologists and nephrologists. Professor Herrington stressed the necessity of active and efficient communication between different medical specialties due to the inherent complexities of CKD. Furthermore, involving patients, their families, and caregivers in multidisciplinary care processes is vital to ensure that treatment priorities align with patient needs, thereby enhancing the overall patient experience and promoting patient-centered care. The guidelines also include a patient-focused version to empower individuals with the knowledge and confidence required for informed decision-making with their healthcare providers. The Task Force Chairs concluded by highlighting that these guidelines represent a significant step towards improving individual and societal health outcomes for those affected by or at risk of CVD and CKD, urging all stakeholders to take note and plan future research to address existing knowledge gaps.
